Information we'll ask for
We'll take your contact details so we can send any forms you might need, and keep you updated. It'd also help if you have the following information about the person who died:
Their full name
Their last known address
Their date of birth
The date they died
Any policy or plan numbers
The death certificate (we might need the certificate number)
If a will has been left, we will need the name of the executor if you know it.

To show you how best to get in touch with us, we need to know what kind of policy they had. Don't worry if you don't know, just choose 'I don't know' from the list below.
For funeral plans taken out with Royal London, where the policy started within the last 12 months please call 0345 646 2094, for any older plans, you will need to contact the funeral provider listed on the certificate of entitlement.
