You can use this form to upload and securely submit an application or instruction form for a workplace pension plan to our servicing team.

We can accept:

  • scanned copies of signed application forms and;
  • applications that have been signed by the customer using an electronic signature (e.g. DocuSign).

The maximum file size is 10MB
This reference will be quoted in an acknowledgement email when you submit the application
You can select more than one file at the same time. You can upload a maximum of 20MB at once, regardless of the number of files
